• Please arrive on time for appointments.
• Please call 24 hours in advance if you must cancel an appointment. There will be a $25.00 charge for appointments not cancelled in a timely manner.
• Please bring all your current medications in their original containers to every appointment.
• Please bring insurance cards and photo identification to each and every appointment.
• Please sign in at the front desk when you arrive.
• Before you leave, please check at the desk to schedule any further appointments and to settle your payment arrangements. All charges are due and payable when services are rendered. There are NO exceptions.
• All children under 18 years of age must be accompanied by a parent or guardian.
• We reserve the right to refuse non-emergent visits to anyone at anytime

At the Hampton Medical Center, we do our utmost to keep to our scheduled appointment times. Scheduled patients will be seen on a priority basis. However, unexpected medical emergencies with other patients may arise that delay your appointment. We will do our best to accommodate patients who have called that day for an appointment or who have walked in for an appointment
